Reasons include: Employees were observed not washing hands between tasks; sanitizer levels at the triple sink were too low; thermometers were missing in the freezer and deep freezer; bulk food containers were not properly labeled; food was open/unprotected in the walk-in cooler and dry storage area; reuse of disposable items was observed; shelves in the walk-in cooler were rusted, in poor condition, dirty, and lined with cardboard; no sanitizer test kit was provided; numerous non-food contact surfaces on the cookline were dirty; the water heater in the dry storage area was leaking; gray water from used mop water was being disposed of on the floor outside the back door; floors and walls behind and under equipment were dirty.
